What to do when a Player has a claim

Excess Accident coverage is automatically provided to registered members of the Florida Youth Soccer Association as part of the registration fee.  The Accident policy provides coverage for registered players during sanctioned events, including regularly scheduled practices and games.  This coverage is excess above the player’s own family health insurance.  There is a $2,000 deductible on each claim. The insurance company will pay 80% of the eligible amount after the deductible has been applied.  The claimant is responsible for  the other 20% of the next $10,000 to a max out of pocket cost of $3000 per injury.

Coverages/Benefits: Accident-Medical $50,000 Maximum; Accidental Death $10,000 Principal Sum; Accidental Dismemberment $10,000 Maximum.
 
Complete the Youth Soccer Accident Proof of Loss (claim form), which is obtained from the Secure section of our website (through your club’s Agent of Record, President or Registrar).  The form must be signed by the Coach, the Agent of Record from your club, and parent or legal guardian.  More information about FYSA insurance can be found here at the Bollinger website.
 
The fully completed, signed claim form must be returned for processing to:

·       Be sure to include any itemized doctor bills as well as all Explanation of Benefits that you receive from your primary insurance carrier.
What keeps the Insurance Company from paying your claim?
 
·       Incomplete or unsigned claim forms
·       “Balance forward statements” from the Healthcare providers rather than Itemized Billings.
·       Failure to provide your insurance company’s explanation of benefits.
 
PLEASE REMEMBER
 
You are responsible for the first $2,000 of your claim and 20% of the next $10,000 to a max out of pocket cost of $3000 per injury.

Goal Safety Reminder

As we progress further into spring and the start of the new soccer season, please remind your coaches and referees to inspect, BEFORE EVERY PRACTICE, TRAINING ACTIVITY, AND GAME (1) all goal posts and nets for safety, that they are properly anchored and secured, and (2) the field and areas around the field that is being used.

Goal Re-Sizing

It has been brought to the attention of FYSA that US Youth Soccer has changed the recommended size of the goals to be used for the U11/12 age group from 7’ x 21’ to 6’ x 18’.

FYSA has no issue if those parties with 7’ x 21’ goals have them re-sized to 6’ x 18’ by any means necessary that then does not comprise the integrity and/or strength of the goals. Any structural work associated with the re-sizing must be performed by a qualified professional to meet or exceed the original manufactures’ specifications.

In addition, all goals must be securely anchored to the ground during any FYSA-sanctioned activity.

The Importance of Properly Anchoring Portable Soccer Goals
The family of a 6-year-old Vernon Hills boy who died after a nearly 200-pound portable metal soccer goal fell on him has agreed to a $2.25 million settlement with the Greater Libertyville Soccer Association, the family's attorney said Friday.

US Youth Soccer Disciplinary Action & Risk Management Report (DAR)
The US Youth Soccer Disciplinary Action and Risk Management Report (“DAR”) is a tool provided by the national organization to track suspensions of individuals participating in US Youth Soccer programs.  This information is updated and provided monthly by the 55 national state associations.  The DAR can be used in conjunction with the FYSA suspension list.  To view the current US Youth Soccer DAR report, click here.
