It is the Affiliate’s responsibility to ensure the 2007/2008 Affiliation Form on file is current. If you have had elections, or for some other reason have any changes that pertain to 2007/2008, please go online and complete a new Affiliation Form. Make sure your changes are for 2007/2008.

 

If you are not specifically named on the 2007/2008 Affiliation Form OR if nobody named on the 2007/2008 Affiliation Form will be attending the upcoming AGM, then a PROXY will need to be completed to designate the name of the person who will have the authority to pick up your club’s credentials. THE ONLY PERSON WHO CAN SIGN this proxy is the AGENT OF RECORD. Do not forget to have the form notarized. Only the original will be accepted; no faxes, scans or emails.

 

This is only for credentials (the packet of information that permits your club to vote at the Sunday morning meeting).

 

The deadline is JULY 1st to register any new players. Get any remaining registrations processed by this date to maximize your voting power at the AGM!
