

PLAYER & COACH MEMBERSHIP
Player Membership
All players applying for membership must do so through an affiliated member club each seasonal-year. FYSA offers the following player member types:
- Competitive Player Membership ($26/player)
- Recreational Player Membership ($13/player)
- TOPSoccer Player Membership ($0/player)

Document Requirements for Players
- Proof of Age/Identity: Competitive Players must submit a proof of age document upon their first competitive registration with FYSA, and a recent head-portrait photo on a regular basis.
- Player SafeSport Requirement: Players that are 18 years old or will be 18 years old by seasonal year end shall complete SafeSport training prior to team assignment.

Coach Registration
All coaches applying for membership must do so through an affiliated member club each seasonal year. Coaches must submit FYSA approved compliance documents and pass a FYSA background check in their GotSport account for verification (if not previously verified).

Coaching Education may be required by a league/competition authority. See local rules for additional information.
- Concussion Education: This requirement is satisfied by completing the CDC Heads Up Course. Coaches may launch the course through their GotSport account. Click here for instructions.
- SafeSport Training: This requirement must be completed on an annual basis. Coaches may launch the course through their GotSport account. Click here for instructions.
- Year One: SafeSport Core Training (1.5-2 hrs)
- Year Two: SafeSport Core Refresher 1 (15-20 mins)
- Year Three: SafeSport Core Refresher 2 (15-20 mins)
- Year Four: SafeSport Core Refresher 3 (15-20 mins)
- Year Five: SafeSport Core Training (cycle repeats)
Background Checks
- Fingerprinting: Florida requires athletic coaches to submit to a Fingerprint Background Check through the Florida Department of Law Enforcement (FDLE) prior to participation. This Fingerprint Based Background Check is known as a FDLE Level 2 Background Check. The FDLE Level 2 Check provides Live Scanning, which alerts the association of criminal charges in real-time.
- National Criminal Check: The SafeSport Code requires associations to conduct a background check every two-years. As a result, FYSA requires a National Criminal Check In between FDLE submission cycles. The National Criminal Check is an online background check through GotSport.
Background Check Schedule
- Year 1: FDLE Level 2 Background Check (Fingerprinting)
- Year 2: National Criminal Check (SSN Based)
- Year 3: No Action Needed
- Year 4: No Action Needed
- Year 5: Resubmit FDLE Level 2 Background Check (Fingerprinting)
Background Check Payment
All Background Checks must be paid for in advance of the background check completion.
- Club Pre-Payment: Member Clubs may prepay for background checks in bulk and provide a voucher code to coaches for redemption and use. Click here for instructions.
- Coach Payment: Individuals coaches & applicants may submit payment for a background check at the time of registration. Click here for instructions.
Completing an FDLE Level 2 Background Check
- Access the club-specific background check link (provided to the Club Agent of Record by FYSA).
- Enter your email address & check your email inbox for a 6-digit code.
- Note: Look for an email with the subject line “ApplicantServices.com Login Code.”
- Input the received 6-digit code on the background check website to begin the transaction.
- Follow the prompts to complete all required sections. To advance from one section to another, click on the “Next” button.
- Find the closest location and select an appointment.
- Note: Different locations will schedule appointments while others may allow for walk-ins. Please read the information specific to the location selected.
- Review, acknowledge, and sign-off the Privacy and Waiver Agreement. Be sure to scroll through the text and document to review and click on the “I Agree” button.
- Upon finishing the online process, expect to receive a confirmation email containing a QR Code and a 7-capital-letter “Payment ID.” Use this email at the fingerprinting appointment. You do not have to print it out, you can present it on your phone.
- Bring two forms of ID to the appointment/walk-in (be sure to read the types of acceptable forms of ID).
- Attend your scheduled fingerprinting session, ensuring you have the QR Code and the 7-letter Payment ID ready to provide to the operator.
After the fingerprint scan is complete:
- Log into the GotSport individual profile.
- Submit a background check in the same manner as previous years (affiliates will only be billed once).
- When individuals are submitting their FDLE Level 2 Background Check (fingerprint), be sure to select the FDLE Level 2 pathway under your clubs’ banner.
