Post Tournament Information and Required Documents
The following items need to be returned to the Florida Youth Soccer Association State Office at the completion of any tournament.
- Red Card Summary
– Due within 72 hours at the end of the tournament.
– Along with the red card summary, you will need to submit the game cards, official rosters and referee supplementals of all players, coaches, or referees that received a red card. - Post Tournament Report
– Due within 15 days at the end of the tournament. - At the conclusion of your tournament, please send scanned copies of your original documents (Post Tournament Report & Red Card Summary) through the Post-Tournament and Red Card Reports Form in GotSport (link below). Notification of Travel
Teams traveling outside of Florida must notify the Florida Youth Soccer Association (FYSA) of their travel plans. This is done by completing the information for either National Travel, Region III Travel, or International Travel. All of these documents are required by FYSA, US Youth Soccer, and the United States Soccer Federation (USSF) in regards to travel. If you have any additional questions about this process, please refer to the videos below for assistance or contact Giancarlo at gdegregorio@fysa.com.
- National Travel items needed:
- US Youth Permission to Travel Form
- Official FYSA Roster (signed by club registrar)
- Approved US Youth Application to Host Form from the tournament you are attending
- All applicable fees paid to FYSA (use link below to submit payment):
– $10.00 = If more than 30 days prior to travel.
– $35.00 = If less than 30 days prior to travel.
– $100.00 = If less than 15 days prior to travel

- These are the South Region (previously Region III) states of US Youth Soccer: Alabama, Arkansas, Florida, Georgia, Louisiana, Mississippi, North Carolina, Oklahoma, South Carolina, Tennessee, North Texas, South Texas.
- You do not need to submit an official permission-to-travel form within the South Region. At the bottom of your official FYSA roster is a statement that gives the team permission to travel within the South Region.

You will submit an application for International Travel to FYSA for conditional approval. Once conditional approval is granted, you will then submit it to US Soccer for their approval. When you have received approval from US Soccer, you will then submit your US Soccer Approval to FYSA for Full FYSA approval. Your permission to travel is not complete until given full approval from FYSA and US Soccer.

– $75.00 = If more than 60 days prior to travel.
– $150.00 = If less than 60 days prior but more than 30 days prior to travel.
– $250.00 = If less than 30 days prior to travel - USSF Fees (Paid Directly to US Soccer when application is uploaded to their portal with FYSA Conditional Approval)

Interstate Permission Form & Guest Player Information
All players coming from another (current year transfer) Youth or Adult State Association must have a release signed by the official registration chairman of the out-of-state association before they will be allowed to register with FYSA. When FYSA receives this interstate permission from the other state, FYSA will register the player.
For players currently registered with FYSA who move to another state and wish to register with that state association, FYSA has the Interstate Permission form posted below. The player’s current registrar must complete this form and then forward to FYSA at jcannon@fysa.com for processing to the other state association.
Players currently registered with another state association who wish to guest play with a Florida Team must first obtain the appropriate permission from their home state association, which must then be forwarded to FYSA’s office.
For players currently registered with FYSA, who wish to guest play with a US Youth Soccer team from another state association, if the play is within the state of Florida, their registrar must complete the FYSA Guest Player form, available in GotSoccer under the club tab. Once completed by the registrar, this form must be attached to the other State Association’s Official State Roster to verify eligibility.
